Need an appointment at our office?
This is NOT dental insurance but rather a dental membership plan. This plan serves as an exclusive mutual 12-month agreement between our patients and our practice. This savings plan cannot be used in conjunction with dental insurance or any other discounts. This plan is ONLY valid at Ellis Dental, located in Kirkwood, MO. Annual plan fees are subject to change.
• In order to enroll in our Smile Membership Plan, your account must have a ZERO balance prior to enrollment.
• The plan is NOT retroactive and will become effective on the date of enrollment for one full benefit year.
• It is the plan member’s responsibility to utilize the services included in the savings plan within their benefit year. Any unused benefits will not be carried over or refunded. The plan is non-transferable.
• In exchange for the care provided under this plan, the covered member agrees to pay all balances in full at the time of treatment. If treatment is not paid in FULL at the time of service, the treatment discount is void.
• The member has the right to opt out of the plan for a full refund within 30 days of enrollment as long as services have not been initiated. If ANY services have been performed or if 30 days from enrollment have lapsed, NO refund will be given. The member will be responsible for paying the remaining balance of the membership plan and treatment regardless of services rendered.
• Services are based upon a plan benefit year. The full plan fee is due on the date of enrollment and eligibility will begin at that time and remain active for one year.
• If appointments are broken without 48 hours prior notice, a cancellation fee will apply.
By enrolling in the Smile Membership Plan, I acknowledge that I have reviewed, understand, and agree to the terms and conditions of the membership as presented by Ellis Dental upon enrollment. I authorize Ellis Dental to process my payment as listed in this agreement.
